A test of plone form gen

This is the Prologue to my first form.  The Registered Field only displays to Managers, however the default value does not get set if the field is not displayed which makes the spreadsheet short one column in the middle.  Need to figure out how to insert value if field is blank.  Adding required to the field doesn't work as expected, anonymous can still submit without a value for the field and the default is not inserted.
